ATHLETIC CLEARANCE
All student athletes must have athletic clearance prior to attending summer camp/league or any tryouts. Please follow the link and directions below:
Athletic Clearance Instructions:
1.) Go to AthleticClearance.com
2.) Register / Create an account under 2024-2025 school year (if you have an existing account- log in and update to 2024-2025. Update any new information and upload a current medical eligibility form)
3.) Print off medical eligibility paper work (THHS Website/Athletics/Athletic Clearance)
4.) Upload the medical eligibility form completed by physician.
5.) MUST be signed, stamped, dated, and have a recommendation by physician (only need to upload the last page- see sample form below)

Steps to get cleared to participate in Athletics at Trabuco Hills High School:
1.) Go to AthleticClearance.com
2.) Register / Create an account
3.) Enter the code given to activate account
4.) Follow steps to input registration data
5.) Print off sports screening paper work if needed (SVUSD_Sports_Screening_Form.pdf)
6.) Upload sports screening paper work signed and stamped by physician (Both pages)
7.) Finalize with electronic parent / student signatures
